Hey Everyone! Thank you for taking the time to fill out this form. We know your time is precious and limited, so we have done our best to consolidate as much as we can into this single form to limit the time required of you. When completed, the information you provide will be transferred to APS where it will then be centrally available for us to make informed decisions around recognition, professional development and overall employee engagement. Please complete all questions (with the exception of the EEOC Data Collection which is voluntary). If a question does not apply to you or you do not have that information available (ie. you have never completed a Strengths Finders assessment), please enter "N/A" as a response. Please note that if you choose to provide information on the last page of this form (titled "EEOC Data Collection - 2019), that information will be used ONLY to complete forms required for reporting to the federal government and will be kept separate from all other personnel records only accessed by the Human Resources Department.
